Please enter your dog's details for entry into the BSDA of GB Open Show on 3rd April 2011.

Payment must be made using Paypal once the form has been submitted.

An online entry fee of £1 will also be charged.  

Payments will be checked to ensure they cover full payment for the classes entered. If there is any discrepancy, a £5 surcharge will be payable. Please ensure that an email address is provided in case a refund is required. Any refunds will be minus the £1 online entry fee.

A maximum of four dogs can be entered in each form submission.  

The online fee will only be charged for the first form submission, any subsequent form submissions for the show will not incur the online fee.

DECLARATION

I/We agree to submit to & be bound by the Kennel Club Rules & Regulations in their present form or as they may be amended from time to time in relation to all canine matters with which the Kennel Club is concerned. I/we also undertake to abide by the Regulations of this Show and not to bring to the Show any dog which has contracted or been knowingly exposed to any infectious or contagious disease during the 21 days prior to the Show, or to bring any dog which has been prepared for exhibition contrary to Kennel Club Regulations for the Preparation of Dogs for Exhibition F (Annex B). I/We agree without reservation that any Veterinary Surgeon operating on any of my/our dogs in such a way that the operation alters the natural conformation of the dog or part thereof may report such operation to the Kennel Club. . I/We declare that where any alteration has been made to the dog(s) natural conformation the relevant permission to show has been granted by the Kennel Club.I further declare that I believe to the best of my knowledge that the dogs are not liable to disqualification under Kennel Club Show Regulations.

NOTE: Dogs entered in breach of Kennel Club Show Regulations are liable to disqualification whether or not the owner was aware of the breach.
